Case Management

 The Behavioral Health Division has licensed social workers available to provide on-going case management for clients with severe and persistent mental health issues who have a court-ordered commitment or settlement agreement. Assessments of an individual’s physical, psychological and social environment may be completed and used to develop strategies that aid in recovery. Case Management is also provided to individuals who receive funding for residential substance use treatment. On-going case management seeks to improve an individual’s personal health, community participation, and continued recovery.
