CLTS

Children's Long Term Support (CLTS) Waiver

 The CLTS program provides community-based support and services for children up to the age of 21 who have substantial limitations in their daily activities and require assistance to remain in their homes and community.


CLTS serves a wide range of children with disabilities within three target group populations: physical disabilities, developmental disabilities, and severe emotional disturbance or mental health. Professional staff can work with families to identify support needs and connect them with community resources. The program focuses on the child’s strengths and family goals.

CLTS is dedicated to providing support and services that are family-centered, inclusive, and empowering while maintaining the child’s independence and dignity.


 Assistance may also be available for:

  • Adaptive aids
  • Communication aids
  • Supportive home care
  • Specialized medical and therapeutic supplies
  • Respite care


You may be eligible for CLTS if your child has a severe disability, is under the age of 21, is living at home, and the child meets a qualified level of care based upon the results of a Functional Screen assessment.

Understanding CLTS-Waiver and Katie Beckett

Katie Beckett provides Medicaid Insurance for children with special/long term care needs when a disability determination is in place.  CLTS-Waiver can provide Medicaid Insurance, but families must also have ongoing service needs.  Both programs require a functional screen to determine functional eligibility.  If you are only looking for Medicaid Insurance, click here.

Information for Providers

 The Centers for Medicare and Medicaid Services (CMS) requires a standardized, statewide claims handling process for providers of Children's Long Term Support Waiver (CLTS-W) services. A Third Party Administrator (TPA) provides a statewide claims process for all agencies administering CLTS-W services, and Wisconsin's statewide claims processing vendor is Wisconsin Physicians Services (WPS). Jackson County will receive and process all invoices related to CLTS expenses. Invoices should be sent to DHHS_Invoices@jacksoncountywi.gov.  Service providers have a four-step process in becoming a registered CLTS-Waiver service provider:


STEP 1 - Review the CLTS Waiver Manual to understand specifics on various service arrays and ensure your services meet the criteria.

STEP 2 -  Please complete the following documents and return them to Jackson County Health and Human Services:

  • Billing and Service Information (TPA)
  • Request for Taxpayer Identification Number and Certificate (W9)
  • Send your completed forms by mail, email, or fax (715-284-7713) to Jackson County Department of Health and Human Services: Attn: Jessica Stinson (jessica.stinson@jacksoncountywi.gov) 421 County Road R, Black River Falls, WI 54615


STEP 3 - Please complete the online registry to register with the State.

  • Children's Long-term Support: Provider Registration  
    • Click on the How to Register tab and complete each section.


STEP 4 - Provide rates and sign a contract. 

  • Jackson County Business Services Department will generate a contract. Once all details have been submitted, and the contract is finalized, services can be authorized.
