CCOP

Children’s Community Options Program (CCOP)

 Jackson County’s Children’s Community Options Program provides a coordinated approach to supporting families who have a child with a disability. The program’s goal is to support, nurture, and facilitate self-determination, interdependence, and inclusion in all facets of community life for the child and family. Our professional staff can assist families in identifying goals, prioritizing and implementing strategies that are flexible, coordinated, and effective to meet the family’s desired outcomes.


These strategies may include:

  • Information, education, and training on advocating on behalf of the child, leadership, and the full array of supports and services available in each community.
  • Methods for connecting families with other families (including parents, guardians, self-advocates, siblings, and grandparents) and support groups for mutual support and networking.
  • Goods and services that promote identified outcomes, benefit the child, and enhance the family’s long-term support roles.


You may be eligible for CCOP if your child has a disability; the child is under the age of 22; and the child meets an eligible level of care based upon the results of a Functional Screen assessment. For more information about CCOP please click here to be re-directed to the Wisconsin DHS Website
